Relationship between pelvic organ prolapse and chronic pelvic pain in women

Abstract
Objective. To study clinical and pathophysiological relationship between pelvic organ prolapse (POP) and chronic pelvic pain syndrome (CPPS) in women, to identify pain development mechanisms, and to consider diagnostic and treatment tactics.
Materials and methods. Scientific publications for the past 10 years, clinical guidelines, and epidemiological data on POP and CPPS were analyzed for a given time frame. Pathophysiological mechanisms (nociceptive, neuropathic pain, central sensitization), POP-Q classification, diagnostic methods (ultrasound, MRI, urodynamics) were studied.
Results. Multifactorial POP-CPPS relationship was identified: chronic pain through mechanical (fascia stretching), compressive (nerve compression), neuroinflammatory mechanisms develop in 30-50% of women with prolapse. Kegel exercises and pessaries are effective in treating this condition, as well as the use of antidepressants (duloxetine, vortioxetine) when CPPS is combined with depressive disorders.
Conclusion. A multidisciplinary, personalized approach is required in the treatment of these conditions. Antidepressants are shown to be effective in treating central sensitization and psychoemotional disturbances accompanying CPPS in POP.
