Представить клинико-морфологическое описание случая гемангиоэндотелиомы селезенки с метастазами в печень и проанализировать ее патоморфологические и иммуногистохимические характеристики с учетом современных диагностических и дифференциально-диагностических критериев.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Представлен случай эпителиоидной гемангиоэндотелиомы (ЭГЭ) селезенки с метастатическим поражением печени у женщины 73 лет, поступившей в стационар с жалобами на боли в левом подреберье, увеличение живота в объеме и желтуху.</p></sec><sec><title>Результаты</title><p>Результаты. При аутопсийном исследовании выявлено массивное новообразование (НО) селезенки размером 15×12×10 см с распространенными метастазами в печень. Гистологически НО характеризовалось двухфазным строением с наличием солидных гнезд эпителиоидных клеток и анастомозирующих сосудистых каналов. Иммуногистохимическое исследование (ИГХ) показало диффузную положительную экспрессию CD31, CD34 и ERG, подтверждающую эндотелиальную природу опухоли, с индексом пролиферации Ki-67 10–15 %.</p></sec><sec><title>Заключение</title><p>Заключение. Данное клиническое наблюдение демонстрирует типичные патоморфологические признаки ЭГЭ селезенки и подчеркивает важность комплексного ИГХ для дифференциальной диагностики редких сосудистых новообразований. Поздняя диагностика на стадии диссеминированного заболевания отражает неспецифический характер клинических проявлений и обуславливает неблагоприятный прогноз. Описание данного случая в русскоязычной медицинской литературе вносит важный вклад в практическое здравоохранение, учитывая крайнюю редкость данной патологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To provide a clinical and morphological description of a case of splenic hemangioendothelioma with liver metastases, and to analyze its pathomorphological and immunohistochemical characteristics in accordance with current diagnostic and differential diagnostic criteria.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A case of epithelioid hemangioendothelioma (EHE) of the spleen with metastatic liver involvement in a 73‐year‐old woman admitted to the hospital with complaints of left upper quadrant pain, abdominal distension, and jaundice was presented.</p></sec><sec><title>Results</title><p>Results. Post‐mortem examination revealed a massive splenic tumor measuring 15×12×10 cm, with widespread metastases to the liver. Histologically, the tumor was characterized with a biphasic pattern, featuring solid nests of epithelioid cells and anastomosing vascular channels. Immunohistochemical analysis demonstrated diffuse positive expression of CD31, CD34, and ERG, confirming the endothelial nature of the tumor, with a Ki‐67 proliferation index of 10–15%.</p></sec><sec><title>Conclusion</title><p>Conclusion. This clinical observation demonstrates typical pathomorphological features of epithelioid hemangioendothelioma of the spleen and underscores the importance of comprehensive immunohistochemical analysis for the differential diagnosis of rare vascular tumors. The late diagnosis at the stage of disseminated disease reflects the non‐specific nature of clinical manifestations and explains the poor prognosis.
