В статье представлен аналитический обзор научной литературы, посвященной эпидемиологии, молекулярной биологии и клиническим проявлениям инфекций, вызванных вирусами герпеса человека 4, 6, 7-го типов, с акцентом на их роль в формировании хронических болевых синдромов у детей. Ключевыми механизмами участия герпесвирусов в генезе хронических болевых синдромов являются: индукция хронического системного и нейровоспаления с выработкой провоспалительных цитокинов; прямой нейротропизм; поддержание центральной сенситизации; запуск аутоиммунных реакций, ведущих к повреждению нервных структур; развитие висцеральной гиперчувствительности через молекулярную мимикрию. Вирусы герпеса человека 4, 6, 7-го типов при их персистенции и реактивации за счет поддержания состояния иммунной дисрегуляции и нейровоспаления создают биологическую основу для трансформации острой ноцицептивной боли в ноципластическую, характерную для формирования хронических болевых синдромов у детей.</p></abstract><trans-abstract xml:lang="en"><p>Human herpes viruses types 4, 6, and 7 are widespread among children, and possess a unique ability for maintaining lifelong latent persistence with periodic reactivation. This article provides an analytical review of the scientific literature on epidemiology, molecular biology, and clinical manifestations of infections caused by human herpesviruses types 4, 6, and 7, with a focus on their role in formation of chronic pain syndromes in children. The key mechanisms of herpesvirus involvement in the genesis of chronic pain syndromes are induction of chronic systemic and neuroinflammation with production of proinflammatory cytokines; direct neurotropism; maintenance of central sensitization; triggering of autoimmune reactions leading to damage to nerve structures; development of visceral hypersensitivity through molecular mimicry.
