На основании анализа данных литературы и клинического случая систематизировать современные представления об этиологии, диагностике и тактике ведения пациентов с эластофибромой спины, а также проиллюстрировать особенности диагностического процесса.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ научной литературы, посвященной эластофиброме. Также представлено подробное клиническое наблюдение за пациенткой 65 лет с двусторонним бессимптомным образованием в подлопаточной области. В диагностике использовались клинический осмотр, магнитно-резонансная томография (МРТ), компьютерная томография (КТ) и гистологическое исследование после биопсии.</p></sec><sec><title>Результаты</title><p>Результаты. Клинический случай продемонстрировал трудности лучевой диагностики: по данным МРТ был заподозрен злокачественный процесс (фибросаркома), тогда как КТ-картина с учетом типичной локализации и структуры образования позволила предположить эластофиброму. Окончательный диагноз был установлен только после гистологического исследования, выявившего фрагментированные эластические волокна («глобулы») и фиброзную ткань без признаков атипии. Учитывая отсутствие симптомов, пациентке была рекомендована выжидательная тактика с динамическим наблюдением. Анализ литературы подтвердил, что современные методы визуализации (ультразвуковое исследование (УЗИ), КТ, МРТ) играют ключевую роль в диагностике, выявляя патогномоничные признаки, а хирургическое лечение показано только при наличии симптомов.</p></sec><sec><title>Заключение</title><p>Заключение. Эластофиброма является редким доброкачественным образованием, часто требующим дифференциальной диагностики со злокачественными опухолями мягких тканей. Наиболее информативными методами диагностики являются КТ и МРТ, но в сложных случаях необходима морфологическая верификация. При бессимптомном течении методом выбора является динамическое наблюдение, так как заболевание не обладает потенциалом к злокачественной трансформации. Представленное клиническое наблюдение подчеркивает важность комплексного подхода, объединяющего данные анамнеза, лучевых методов и морфологического исследования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To systematize modern concepts of the etiology, diagnosis, and management of patients with elastofibroma of the back, and to illustrate the features of the diagnostic process based on the analysis of literature data and a clinical case.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An analysis of scientific literature dedicated to elastofibroma was conducted. A detailed clinical observation of a 65-year-old female patient with bilateral asymptomatic masses in the subscapular region was also presented. Diagnosis involved clinical examination, magnetic resonance imaging (MRI), computed tomography (CT), and histological examination after biopsy.</p></sec><sec><title>Results</title><p>Results. The clinical case demonstrated the difficulties of radiological diagnostics: according to MRI data, a malignant process (fibrosarcoma) was suspected, whereas the CT picture, considering the typical location and structure, suggested elastofibroma. The final diagnosis was established only after histological examination, which revealed fragmented elastic fibers (“globules”) and fibrous tissue without signs of atypy. Taking into account absence of symptoms, the patient was recommended a wait-and-see approach with dynamic observation. The literature analysis confirmed that modern radiological methods (ultrasound, CT, MRI) play a key role in diagnostics, revealing pathognomonic signs, and surgical treatment is indicated only in the presence of symptoms.</p></sec><sec><title>Conclusion</title><p>Conclusion. Elastofibroma is a rare benign formation, often requiring differential diagnostics with malignant soft tissue tumors. The most informative diagnostics methods are CT and MRI, but verification is only possible histologically in complex cases. In asymptomatic cases, dynamic observation is the method of choice, as the disease lacks potential for malignant transformation.
