Цель исследования. Продемонстрировать клинический случай аденомы надпочечника со смешанной гормональной секрецией.

Материалы и методы

Материалы и методы. Ретроспективный анализ истории болезни пациентки, находившейся на стационарном лечении в эндокринологическом отделении ГУ «Республиканский научно-практический центр радиационной медицины и экологии человека».

Результаты

Результаты. Эндокринные артериальные гипертензии являются актуальной проблемой современной эндокринологии. Большая часть данных артериальных гипертензий относится к первичному гиперальдостеронизму (ПГА), представленному аденомой или двусторонней гиперплазией надпочечников. Диагностика данных состояний представляет трудности: требуется подтверждение наличия объемного образования по данным инструментальной диагностики, а также лабораторного подтверждения его гормональной активности. В процессе исследования врач-клиницист может обнаружить редкие особенности, свидетельствующие о смешанной гормональной секреции доброкачественных образований надпочечников.

Заключение

Заключение. Первичный гиперальдостеронизм и эндогенный гиперкортицизм являются частыми причинами развития вторичных артериальных гипертензий, которые ухудшают качество жизни пациентов и увеличивают риски смертности и инвалидизации. Встречаемость аденом со смешанной секрецией — крайне редкий случай в эндокринной практике, не влияющий на дальнейшую тактику ведения пациентов, но представляющий особые трудности для диагностики.

Objective. Demonstration of a clinical case of adrenal adenoma with mixed hormonal secretion.

Materials and methods

Materials and methods. Retrospective analysis of the medical history of a patient who was hospitalized in the endocrinology department of the Republican Scientific and Practical Center for Radiation Medicine and Human Ecology.

Results

Results. Endocrine arterial hypertension is a current problem of modern endocrinology. The majority of the arterial hypertension data is attributable to primary hyperaldosteronism, represented by adenoma or bilateral adrenal hyperplasia. Diagnosis of these conditions presents difficulties, requiring confirmation of the presence of a volumetric formation according to instrumental diagnostics, as well as laboratory confirmation of its hormonal activity. A clinician may find rare features indicating mixed hormonal secretion of benign adrenal lesions during the study,

Conclusion

Conclusion. Primary hyperaldosteronism and endogenous hypercortisolism are common causes of the development of secondary hypertension, which worsen patients’ quality of life and increase the risks of mortality and disability. The occurrence of adenomas with mixed secretion is extremely rare in endocrine practice, which does not affect the further management of patients, but poses special difficulties for diagnosis.
