Цель исследования. Продемонстрировать клинический случай атипичного течения анкилозирующего спондилита (АС).

Материалы и методы

Материалы и методы. Проведен ретроспективный анализ истории болезни пациента, находившегося на стационарном лечении в ревматологическом отделении государственного учреждения здравоохранения «Гомельская городская клиническая больница № 1» ( ГУЗ «ГГКБ № 1»).

Результаты

Результаты. Анкилозирующий спондилит относят к наиболее трудно диагностируемым причинам специфической боли в нижней части спины. Медико-социальное значение заболевания заключается в высоком уровне инвалидизации людей молодого возраста и снижении качества жизни пациентов. Специфические диагностические маркеры спондилита отсутствуют на ранней стадии заболевания. Причинами поздней диагностики заболевания являются несвоевременное обращение пациентов за медицинской помощью, малосимптомное течение АС, недостаточное знание врачами диагностических критериев заболевания. В статье представлен клинический случай атипичного течения АС у пациента молодого возраста, протекающего без признаков системного воспаления и классических проявлений заболевания.

Заключение

Заключение. Целенаправленное исследование пояснично-крестцового отдела позвоночника и крестцово-подвздошных сочленений (КПС) должно быть рекомендовано пациентам с болевым синдромом в нижней части спины с целью верификации АС.

Objective

Objective. To demonstrate a clinical case of atypical ankylosing spondylitis.

Materials and methods

Materials and methods. Retrospective analysis of the medical history of a patient hospitalized in the Rheumatology Department of Gomel City Clinical Hospital No. 1.

Results

Results. Ankylosing spondylitis is one of the most difficult to diagnose causes of specific pain in the lower back. The medical and social significance of the disease lies in a high level of disability of young people, and in a decrease in the quality of patients’ life. There are no specific diagnostic markers of spondylitis at an early stage of the disease. The reasons for the late diagnosis of the disease are the untimely arrival of patients for medical help, the asymptomatic course of ankylosing spondylitis, and insufficient knowledge of the diagnostic criteria of the disease by doctors.

The article presents a clinical case of atypical ankylosing spondylitis in a young patient with no signs of systemic inflammation and classic manifestations of the disease.

Conclusion

Conclusion. Targeted examination of the lumbosacral spine and sacroiliac joints should be recommended for patients with pain in the lower back in order to verify ankylosing spondylitis.
