Изучить особенности структурно-геометрического ремоделирования сердца у пациентов с сахарным диабетом (СД) в зависимости от стадии хронической болезни почек (ХБП).</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. Обследовано 449 пациентов (127 мужчин и 322 женщины) с СД 1 типа (133 человека) и СД 2 типа (316 человек) в возрасте 62 [52; 69] лет. Группу сравнения составили 65 случайно отобранных лиц (54 женщины и 11 мужчин) без нарушений углеводного обмена и поражения почек из числа добровольцев в возрасте 59 [46; 66] лет. Лабораторное обследование пациентов включало определение в сыворотке крови креатинина, расчет скорости клубочковой фильтрации (рСКФ) по формуле CKD-EPI. Выполнялось эхокардиографическое исследование с изучением структурно-геометрических показателей сердца. Гипертрофию левого желудочка (ГЛЖ) диагностировали при значениях индекса массы миокарда левого желудочка (ИММЛЖ) у мужчин более 115 г/м2, у женщин — более 95 г/м2.</p></sec><sec><title>   Результаты</title><p>   Результаты. ГЛЖ выявлена у 57,7 % пациентов с СД, в группе сравнения — у 35,4 %. Частота ГЛЖ возрастала при снижении рСКФ и диагностировалась в 100 % случаев при ХБП 5. У пациентов с СД значимо преобладал вариант ремоделирования левого желудочка по типу концентрической ГЛЖ (29,2 %). Множественный линейный регрессионный анализ продемонстрировал независимое от участия других факторов влияние на величину ИММЛЖ возраста (ß = 0,30, p = 0,01) и концентрации креатинина сыворотки крови (ß = 0,15, p &lt; 0,0001), что подтверждает их самостоятельное патогенетическое значение.</p></sec><sec><title>   Заключение</title><p>   Заключение. Таким образом, у пациентов с СД независимо от стадии ХБП преобладающим вариантом ремоделирования ЛЖ является концентрическая ГЛЖ. Изучение особенностей ремоделирования миокарда у пациентов с СД и ХБП имеет важное значение при определении сердечно-сосудистого прогноза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Objective</title><p>   Objective. To study the features of structural and geometric cardiac remodeling in patients with diabetes mellitus (DM) depending on the stage of chronic kidney disease (CKD).</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. We examined 499 patients (127 men and 322 women) with type 1 DM (133 people) and type 2 DM (316 people) aged 62 [52;69] years. The comparison group consisted of 65 randomly selected volunteering individuals (54 women and 11 men) without disorders of carbohydrate metabolism and kidney damage aged 59 [46;66] years. The laboratory examination of the patients included the measurement of serum creatinine, eGFR according to the CKD-EPI equation. An echocardiographic investigation was performed to study structural and geometric heart parameters. Left ventricular hypertrophy (LVH) was diagnosed when the left ventricular myocardial mass index (LVMI) exceeded 115 g/m2 in men and 95 g/m2 in women.</p></sec><sec><title>   Results</title><p>   Results. LVH was detected in 57.7 % of patients with DM, while in the comparison group – in 35.4 %. The frequency of LVH increased with a decrease in eGFR and was diagnosed in 100 % of cases with CKD 5. In patients with DM, the left ventricular concentric remodeling significantly predominated (29.2%). Multiple linear regression analysis demonstrated the influence of age (ß=0.30, p=0.01) and serum creatinine (ß = 0.15, p &lt; 0.0001) on the LVMI, independent of other factors, which confirms their independent pathogenetic significance.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. Thus, patients with DM regardless of the CKD stage have structural and functional changes in the LV myocardium. Regardless of the nitrogen-excretion function of the kidneys, the predominant pattern of LV remodeling is concentric.
