Изучить первичную резистентность H. pylori к кларитромицину жителей Гомельской области методом полимеразной цепной реакции в реальном времени (ПЦР РВ) с использованием TaqMan® MGB-зондов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включено 184 пациента с диагнозом гастрита и дуодениа, K29, медиана возраста — 48,5 года (25 и 75 % — 37 и 61 год). Согласно анкетным данным пациентов, эрадикационная терапия с применением кларитромицина не проводилась. Для определения резистентности H. pylori к кларитромицину использовали метод ПЦР РВ с применением TagMan® MGB-зондов.</p></sec><sec><title>Результаты</title><p>Результаты. Все 184 исследуемых образца ДНК были положительными по гену Rnase P (ВКО) и учитывались при дальнейшем анализе (Ct, HEX 20,20–34,14). ДНК гена cagH (Ct, FAM 21,26–33,04), свидетельствующая об инфицировании бактерией, подтверждена в 152 образцах (82,6 %). ДНК гена 23SrRNA (точечные мутации A2142G и A2143G) выявлена в 16 из 152 образцов ДНК — 10,5 % (Ct, Hex 20,24–31,17). Заведомо положительные контрольные пробы имели характерный рост кривых по соответствующим каналам детекции, в заведомо отрицательных — рост кривых не отмечен.</p></sec><sec><title>Заключение</title><p>Заключение. Первичная резистентность Н. pylori к кларитромицину у жителей Гомельской области составила 10,5 %, и применение тройной эрадикационной терапии первой линии, включающей ИПП, амоксициллин и кларитромицин, в качестве эмпирической в данном регионе согласуется с рекомендациями Mаастрихт III-VI и постановлением Министерства здравоохранения Республики Беларусь от 01.06.2017 № 54: клинический протокол «Диагностика и лечение пациентов с заболеваниями органов пищеварения». Применение ПЦР РВ с использованием TaqMan® MGB-зондов обосновано для определения резистентности H. pylori к кларитромицину, назначения индивидуализированного лечения и оценки эффективности схем эрадикации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To study primary resistance of H. pylori to clarithromycin in residents of Gomel region by real-time polymerase chain reaction (PCR RT) using TaqMan® MGB probes.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 184 patients diagnosed with gastritis and duodenitis, K29, median age 48.5 years (25% and 75% were 37 and 61 years old). According to the patients’ questionnaires, no clarithromycin-based eradication therapy was administered. To determine the resistance of H. pylori to clarithromycin, a PCR RV method using TagMan® MGB probes was used.</p></sec><sec><title>Results</title><p>Results. All 184 tested DNA samples were positive for the Rnase P gene (ICS) and were considered in further analysis (Ct, HEX 20.20-34.14). DNA from the cagH gene (Ct, FAM 21.26-33.04), indicating infection with the bacterium, was confirmed in 152 samples (82.6%). DNA from the 23SrRNA gene (point mutations A2142G and A2143G) was detected in 16 of 152 DNA samples 10.5 % (Ct, Hex 20.24-31.17). The positive control samples had characteristic curve growth in the corresponding detection channels; no curve growth was observed in the negative samples.</p></sec><sec><title>Conclusion</title><p>Conclusion.
